Our Company

 

Who we are: Community Health Ventures, is wholly owned by the non-profit Muskegon Community Health Project. Both are located in Muskegon County Michigan. MCHP was created through a W. K. Kellogg Foundation grant to the Community Foundation For Muskegon County in 1994 for the purpose of developing community driven solutions to health related problems. The WKKF grant required that MCHP develop sustainability beyond the life of the grant. As a result, Community Health Ventures was created in 2001 to market consulting services and software products to other communities as a means of providing a sustainable revenue stream to MCHP, as well as promoting the philosophy of sharing successful programs with communities having similar needs.

Community Health Ventures provides technical assistance, consulting, and resource tools to communities working collaboratively to improve access to health care and case management for specific high-risk groups. These high-risk groups typically consume most of the community revenue for safety net programs. CHV enables communities to better manage the cost of safety net services for these groups and improve outcomes through collaborative community-based programs.

What we do: CHV has developed a core group of expert consultants in the field of community collaborative solutions. CHV consultants have diverse backgrounds with expertise in designing, developing, and implementing collaborative community solutions. The CHV consultants frequently include experts from one community teaching best practices to another community with similar problems.

CHV has helped communities across the nation with health related problems involving indigent health care; health coverage for the working uninsured; and collaborative treatment for juvenile offenders. The support to clients provided by CHV includes technical assistance for; community planning, development of public policy, marketing and sales of community programs, and web-based software for the collaborative community case management of at-risk populations.

CHV has also developed partnerships with technology experts capable of supporting the software applications developed by CHV to improve the community collaborative case management of high-risk groups. Successful community collaborative programs require technological means for sharing information and tracking outcomes. This technology must be affordable to even the smallest communities, and yet scalable to meet the needs of larger communities. CHV has developed technology solutions that have been replicated elsewhere with strong positive results.
